I continue to cover in more detail things which are important when considering Preparation for Adulthood. This week is all about person centred planning.

听Person Centred Planning

What is a Person-Centred Plan? It is a process which puts you (the person) at the centre of planning and decisions which affect you.

It is in line with a phrase linked to the NHS 鈥渘o decision about me without me鈥�.

Who should have a person centred plan? A child or adult with a disability or older person will benefit from a plan, which is a process for identifying and organising the support they require.

Who should be involved?

• The young person; this is their plan and belongs to them.
• Their parents or other close family members who know them best.
• Close friends or other family members who know them and their family well.
• Healthcare, social care and educational professionals who already support them.

What are the 5 key elements of a person centred plan? (Sanderson 2000)

1. The person is at the centre
2. Family members and friends are supporters in planning
3. The plan reflects what is important to the person, their capacity and the support they require
4. The plan results in actions that are about life, not just services and reflect what is possible rather than what is available
5. The plan results in ongoing listening, learning and further action

What is the process?

• Information gathering and professionals who support them, from the young person to those who know them best
• Focus on strengths, who is important to them, what they like doing, what are their aspirations.
• Develop a plan that is forward thinking.

Why is a Person-Centred Plan important for our students?

• It ensures that the young person鈥檚 voice is heard, that they have control of the plan.
• It helps parents/carers to think about life after education.
• It focuses on the young person, their strengths, what is working well or not so well, who and what is important to them.
• It helps the young person, their parents/carers and professionals form a plan which is aspirational and identifies the support needed for it to happen.

Mochii: (Multiple Organisations harnessing inclusivity and independence)

I鈥檝e just seen this online and wondered if it would be of interest to any of our families.

鈥�4-7pm is our Wednesday Teens Group

(Times are not strict so if you cant make it until 4:30-5 no problem however the session will still cost 拢12 due to the hire cost of the hall)

A fun-filled gathering for teenagers aged 12-17 with low-medium needs, designed to build confidence, independence, community and foster positive relationships.

We have plenty of Board Games, Video Games, Pool, Table Football as well as Arts and Crafts and a whoolleeeeee storage room filled with plenty of toys to play with at your hearts content

We will also have a small tuck shop with drinks, crisps, sweets and chocolate bars available for purchase.

Siblings also welcome and diagnosis not required.

All staff are DBS checked and trained in safeguarding and first aid.

They are thinking of doing a post 18+ group too.

Thanks to those who attended our meeting on Monday. We鈥檝e been advised that a larger, foundation wide event will be taking place in June and that FoPHS will be allocated space to run our event. I鈥檒l share more details as they become available. It means that, if this goes ahead it will replace our biennial fayre.

We have purchased an annual school metro pass which can be used when classes are going into the community. This was very well used last year, often for social community trips.

School photos: We discussed the school photos which, last year were taken and edited by one of our wonderful teachers, Beth who is a keen amateur photographer. This year, we are asking for an upfront payment with a suggested minimum donation of 拢5, although we know that many of you choose to pay more, which is greatly appreciated.听 This is to help us ensure that Beth鈥檚 time is used as effectively as possible and that only those of you who want photos of your children receive them.听 Of course, if you are not happy with your photos, you will be given a full refund.

Easter: FoPHS will be funding an Easter Egg or sensory alternative for every student again this year. We will also support the MOVE Easter event with resources.
